With Canada’s aging population, we recognize the need to focus on how health care providers serve the nation’s older adults. At Trillium Health Centre, this is a key opportunity in our three-year Strategic Plan.  Canadians are demanding we improve access and service delivery through enhanced integration, coordination, communication and client-focused care.  We are moving forward on our commitment to transforming how care is provided to seniors within our health centre, as well as between providers and beyond.

All individuals and organizations with a shared desire to improve seniors’ care, either within their organizations, between organizations or beyond, are invited to our two-day conference/think tank to improve seniors’ health today for tomorrow.

This conference, scheduled during Seniors’ Month, on June 23, 24, 2010, will bring together international, national and local experts and thought leaders to develop a strategy and action plan for integrated care systems for older adults within and between the various sectors of the health care system.
